Dear Parent or Guardian:

 ACCESS Family Care Portable Dental Unit will be at our school to provide general dentistry the week of April 16-20. This program is available for all students, but please be sure to have your child’s paperwork turned in by the week prior to participation in order to have them participate in the program. ACCESS Family Care WILL file MO HealthNet and/or  private Dental Insurance. If your child does not qualify for MO HealthNet, but you have applied, please include a copy of the rejection letter. Also be sure to complete the Patient  balance Assistance form, which qualifies your child to receive monetary assistance through charity grant funds for the balance of the patient’s (students) bill when receiving in-school portable dental services. If this form is not completed the remaining balance will be billed to the parents address.

 If you would like your child to receive dental services during school hours, please complete the medical history form, and include a copy of your Mo HealthNet/Insurance card. Return all documents to the school via your child, front office or school nurse to be given to ACCESS Family Care staff upon arrival at the school.

 ACCESS Family Care is committed to serving the community in non-traditional locations, realizing it is often difficult for parents to take time off work or find transportation to bring their child to the dentist.

 Portable dentistry allows in-school dental services which reduce the amount of class time a child may miss from conventional dentist appointments, increases learning capacity by reducing mouth pain and teaches children about routine dental care. Portable dental services include; exams, x-rays, cleanings, sealants, fillings, extractions, stainless steel crowns, and in some cases pulpotomies. All services are performed by a licensed General Dentist, and Registered Dental Hygienist. If they are unable to complete your child’s dental care while in the school setting, the student will be referred to one of the clinics for follow-up treatment. Also, if your child requires extensive treatment by a specialist, a referral will be made.

Please note that general dentistry (exam, x-rays, cleanings) can only be performed once every 6 months. If your child has a regular dentist, please continue treatment with that dentist. ACCESS is not responsible for any charges that may result from your child being seen by another dentist sooner than 6 months. ACCESS, however can obtain dental records that will allow their providers to see when your child is due for their next appointment. If you have any questions or concerns, please call the school and ask to speak to the school nurse.
